For 7th and 8th Grade Regular Program
To complete enrollment the following items must be brought to the Mark West Charter School office:
- Signed Student Registration Form
- MWCS Student and Parent Pledge
- Parent/Guardian Application Questionnaire and Student Application Questionnaire
- Student Profile must be submitted by the student’s current teacher
- Jupiter Grade Form
- Birth Certificate
- Immunizations
- Attendance Records
- Discipline Records
